Can I use just one half?

Is it safe to hold against skin?

The top setting reaches about 126°F. It is fine to hold, but do not press it against bare skin for long stretches. Use the low setting for children and for anyone with reduced sensation.

Can I fly with it?

Yes, in carry-on luggage. The batteries are small and well under the 100 Wh limit airlines use for portable electronics. Keep them in your cabin bag, not checked luggage.
